To complete your application, you will need to enter the information as detailed on the vaccination card issued by the health team and attach a passport size photo, valid State issued ID (back and front of your ID cards/Driver's Licence Or Bio-data page of passport) and the back and front of your Vaccination Card.

I understand that the digiGov System will store my Bio-data (full name, date of birth, address, contact information, etc.) and Covid-19 Vaccination information gathered about me for the sole purpose of:

Generation of the digital certificate
Verification of the authenticity of the certificate and information contained on the certificate.

I understand and acknowledge that any information shared with the digiGov System is for the sole purpose of generation of the Covid-19 digital Vaccination certificate and shall not be shared with any third party without my explicit consent.
